The purpose of this subcontract with Ketchum, Inc. is to provide research support in the planning, development, execution and monitoring of communications activities for the From Coverage to Care (C2C) Initiative. C2C seeks to help consumers understand their coverage, connect to the care they need, create changes in the health care system that improve access to care, as well as inform consumers, stakeholders, and health care professionals of the benefits of chronic care management services for individuals with chronic care needs.
